NEW AND EXISTING PATIENT POLICIES
Harbor Interventional Pain Center, LLC strives to provide our patients with safe, efficient and pleasurable care. Please review the following policies provided for you. Let us know if you have any concerns or questions. We will email, fax or mail a copy of our policies upon request.
IDENTIFICATION
New patients are required to bring a photo ID, and insurance card(s) to their appointment.
CO-PAYS
Co-pays are due at the time of service.
CANCELLATION/APPOINTMENT NO SHOW
24-hour notification of cancellation is required. If a patient fails to give 24-hour notice of appointment cancellation and/or does not present for their appointment, a charge of $25.00 for an office visit and $75.00 for missed interventional procedure appointment(s) will be applied to their account.
CONTROLLED MEDICATIONS
All patients that are prescribed opioid and/or controlled medications from Harbor Interventional Pain Center, LLC will be required to read, sign, understand and strictly abide to our controlled medication(s) contract. Patients that are prescribed opioid and/or controlled medications will be subject to random urine drug screening and routine/random pill counts. Patients who are found to be non-compliant or in violation of their controlled medication contract will be discharged from the practice.
URGENT AND EMERGENCY MATTERS
If an urgent (but non-emergency) matter occurs after hours, please call the main office phone number at (603) 294-9190 and leave a message at the prompt. The physician on-call will return your call as soon as possible.
If your situation is an emergency, call 911 or go to the Emergency Room that is located closest to you.
BEHAVIOR
Unprofessional behavior towards our staff, other patients and visitors will not be tolerated. Inappropriate language or behavior will result in being discharged from our practice.
PROCEDURE POLICY
Patients who are scheduled to have an interventional procedure will need to arrive 15 minutes before their appointment time.
SEDATION POLICY
If you are receiving sedation for a procedure, you MUST be accompanied by a responsible adult who will be required to remain on the premises during your procedure and drive you home. PLEASE NOTE: Your escort that is responsible for driving you home MUST be with you when you check in at the front desk. For your safety, you will not be discharged to an Uber, Lyft, Taxi or other type of public transportation. Failure to follow this policy will result in your procedure being cancelled and rescheduled.
URINE DRUG SCREENING
All patients who receive controlled medications are subject to random urine drug screenings. Patients are responsible for all co-pays, deductibles, and or coinsurances for this service. Urine drug screening is performed outside of our office at a nationally certified lab. This requires you to go to the lab for urine drug screening. The results will be sent to Harbor Interventional Pain Center, LLC.
INSURANCE
It is the patient’s responsibility to contact our office and notify us of any changes to your insurance PRIOR to your appointment. Waiting until the day of your appointment may result in a delay of your appointment. As a courtesy our office will check eligibility. However, it is ultimately the patient’s responsibility to be aware of and track this information.
REFERRALS
Some insurances require an insurance referral from your Primary Care Physician or other healthcare provider to be seen by a specialist. It is the patient’s responsibility to track and contact their referring physician when a referral, and or an updated referral is required by their insurance provider. Referrals must be provided before your appointment.
Please note: It is the patient’s responsibility to contact their insurance provider to verify if their insurance provider requires a specialist referral to be seen at Harbor Interventional Pain Center. In some cases, insurance providers will refuse payment for services when the proper referral process is not followed. Not following this process can result in the patient being financially responsible for services rendered at Harbor Interventional Pain Center. Referrals can be faxed to our secure medical records fax at (833) 983-2933.
INSURANCE AUTHORIZATIONS
Select insurance companies require prior authorizations for procedures and/or medications. We start prior authorizations for procedures before the procedure is scheduled. We start prior authorizations for medications once the pharmacy advises our office that a medication prior authorization is required. Please be sure to inform us if you receive a notice that a medication prior authorization is required, so that we can start the process for you.
Our office will call you to schedule procedure(s) once the prior authorization for the procedure is approved. If a prior authorization is denied, patients will be notified as soon as possible.
Please note that physical therapy documentation may be required for procedure, imaging and medication prior authorizations. Failure to comply with the recommended treatment plan could result in denials of appointments and medication request(s).
NARCAN
Narcan is a standard of care for patients receiving chronic opioid therapy. Narcan, also known by the generic name naloxone, is an opioid reversal drug which will be prescribed as a rescue medication should the patient accidentally overdose.
